LINC, Inc. has implemented the Unite Us platform to facilitate coordinated care between local agencies, while protecting client confidentiality and lessening trauma. The Unite Us platform streamlines and enhances the coordination of care among nonprofits and direct service agencies. This innovative technology enables LINC to connect its clients seamlessly to a network of community-based services, ensuring that each individual receives the most comprehensive client-centered support possible. By integrating this platform, LINC effectively manages referrals, tracks client progress, and facilitates more efficient communication between service providers — all while maintaining the highest standards of client confidentiality.

With the Unite Us platform, LINC can reduce redundancies and alleviate the repetitive retelling of personal histories that often retraumatizes individuals seeking support. Clients now only need to provide their information once, and through Unite Us, LINC can securely share the necessary details with relevant service providers. This helps prevent the stress and emotional toll of repeated disclosures, especially for clients with complex needs. By minimizing these triggers, LINC strengthens its commitment to trauma-informed care and provides a more supportive, empathetic environment for all.
