What We Do

 

NC FIT at LINC, Inc. is a partner in the North Carolina Formerly Incarcerated Transition (NC FIT) Program, sponsored by UNC Family Medicine. The program is staffed by community health workers who work on a case-be-case basis to connect justice-involved individuals and formerly incarcerated citizens with primary healthcare providers. The community health workers in the NC FIT program maintain resources for the treatment of chronic disease, mental illness and/or substance use disorder.

Who is Eligible for NC FIT Services

 

  • Individuals who are justice-involved
  • Individuals who are formerly incarcerated
  • New Hanover County residents (Pender and Brunswick County residents on a case-by-case basis)
  • Any gender
  • Ages 18+

How to Apply

 

NC FIT candidates can be referred by a healthcare professional, parole or probation officer, or other non-profit case worker.

Individuals who qualify for NC FIT services may also contact the program directly.

 

All NC FIT participants will be required to sign up for MEDICAID, unless they have current health insurance. Medicaid applications can be submitted online at North Carolina ePASS

Medicaid Expansion in North Carolina has made benefits available to those who may not have qualified in the past.

Adults ages 19 or over and under age 65 may now qualify for Medicaid if:

– You are not receiving/not entitled to Medicare,
– You are not pregnant,
– Your income is below the income threshold of 138% of the Federal Poverty Level,
– You are an adult without a child(ren) or
– You are the parent or caretaker of a child(ren) in the home under the age of 21, that child(ren) must be covered by comprehensive health insurance (private or Medicaid).
